Please note:
(1) “My partner” means anybody who meets the definition in the Localism Act 2011, i.e. a spouse or civil partner or a person with whom you are living as if a spouse or civil partner.
You should detail:
(a) the nature of every employment, business, office, trade, profession or vocation that you or your partner have, with a short description of the job concerned.
(b) the name and address of the employer.
(c) if you or your partner are employed by a company, the name and address of the company paying your or their wages or salary, not that of the ultimate holding company.
(d) the name and address of any company for which you or your partner are a paid director.
(e) the name and address of any firm in which you or your partner are a partner.
(f) where you or your partner hold an office, the name of the person or body which appointed you or your partner.
